The RDW lever, and what the research says so far
RDW was the strongest accelerated aging signal in this study, and it is one of the few CBC values with a plausible path to improving it. Early research on C15:0 (pentadecanoic acid, an odd-chain saturated fat) links higher levels to stronger, more stable red blood cells, and preclinical work suggests supplementing it lowers RDW. I want to be straight with you about the evidence: it is promising, not proven, and mostly preclinical so far. If you want to explore C15:0, fatty15 is the pure supplement form.

What Is Biological Age?
Your chronological age counts birthdays. Biological age tries to answer a more useful question: how old does your body act? Two 55 year olds can carry different disease risks, recover at different speeds and age on different trajectories, and chronological age captures none of that.
Researchers estimate biological age by finding measurements that drift predictably as we get older, then checking whether your values look older or younger than expected for your age. Some methods read DNA methylation (the epigenetic clocks), some use panels of blood chemistry and some rely on fitness tests or questionnaires. The HemeAge Study: 163,000 People, One Routine Blood Test
In early 2026, cardiology researchers at Houston Methodist asked whether a plain CBC could work as an aging clock. They trained a machine learning model on blood counts from 53,355 Americans in NHANES, the CDC survey that pairs health exams and lab work with a nationally representative sample, teaching it to predict each person’s age from 19 standard CBC values. They named the resulting estimate HemeAge, then applied it to 109,844 hospital patients and sorted everyone into three groups based on the gap between predicted and actual age.
- Resilient: blood reads more than 10 years younger than your age
- Proportionate: blood reads within 10 years of your age
- Accelerated: blood reads more than 10 years older than your age

What they found was striking. After adjusting for age, sex, race, existing conditions and social factors, accelerated agers had about three times the risk of dying over follow-up compared to proportionate agers, plus a 37 percent higher risk of major cardiovascular events. Resilient agers had a 41 percent lower death risk. This is an observational study, so it shows association rather than cause, but the pattern was strongest in midlife (ages 40 to 80) and held for the separate outcomes of heart failure and heart attack. Stroke on its own did not reach statistical significance.
The strongest single signal was RDW, red cell distribution width, which measures how much your red blood cells vary in size. That finding fits a long line of evidence and is what the free biological age calculator weighs heavily. A meta-analysis of 11,827 older adults in the Journals of Gerontology found that every one percent increase in RDW carried a 14 percent higher mortality risk and the association held even in people without major disease. Higher MCV and lower red blood cell counts also pushed predicted age upward.

How I Built This Free Biological Age Calculator and What It Cannot Tell You
The study authors could not release their code, which runs on protected hospital records, so I rebuilt the model from their published protocol: the same machine learning method, the same 19 CBC inputs, trained on the same public NHANES survey the original team used. Two differences worth naming. I could reach the 1999 to 2006 survey cycles rather than their full 1999 to 2010 span and I trained on adults only, since that is who uses a calculator like this. My version still lands close to theirs. RDW and MCV came out as the two strongest aging signals, matching what they found and my typical prediction error runs about 17 years against the 15.8 years they reported. Every number, setting and data source is in the methods box below, so anyone who wants to check my work can.
Here is what that accuracy means in plain terms. Blood counts only loosely track age. In the study and in my replica alike, typical prediction error runs 15 or more years and predictions drift toward the middle of the age range. That drift produces a pattern that looks backwards at first: the study’s resilient group averaged 68 chronological years while accelerated agers averaged just 30. This makes sense once you see the mechanism. Because predictions pull toward the middle, a 70 year old’s blood tends to read younger than she is and a 30 year old’s tends to read older, so older people got sorted into the resilient bucket far more often. To keep your result honest, the calculator also compares your reading to the typical prediction for someone your age, which is the right question: does your blood read older or younger than your peers?
One CBC is a snapshot. Hydration, a recent illness, altitude and even lab equipment can affect these values. Treat the number as a trend to watch across your annual labs rather than a verdict and bring anything outside your lab’s reference range to your doctor.

Training data. NHANES survey cycles 1999-2006, via the harmonized public dataset published by the Patel Lab at Harvard (available on GitHub). Adults 18 and older with all 19 CBC values present: 19,994 people. The original study used cycles 1999-2010, all ages, 53,355 people. I could not access the 2007-2010 cycles and restricted to adults because that is who uses this calculator.
Model. XGBoost regression predicting chronological age from the study's 19 CBC variables, matching the paper's method. Settings: 200 trees, maximum depth 4, learning rate 0.08, 80 percent row and column subsampling. Trained on a random 80 percent of the sample with 20 percent held out for testing.
Accuracy on held-out data. R² 0.29, root mean square error 17.1 years, mean absolute error 14.0 years. The published model reported R² 0.20 and RMSE 15.8 years on its NHANES population. Top predictors in my model, in order: MCV, RDW, lymphocyte count, red blood cell count, neutrophil count. The paper reported RDW first, then MCV, neutrophils and RBC, so the same four markers lead in both with minor reshuffling.
Categories and the age-adjusted view. Resilient, Proportionate and Accelerated use the study's exact thresholds (a gap of more than 10 years in either direction). The age-adjusted comparison comes from the model's average out-of-sample prediction at each age, computed by 5-fold cross validation so the curve reflects how the model treats people it has never seen. The percentile assumes readings spread around that average with a standard deviation of 9.3 years, which is what the cross validated data shows. A CBC entered without its differential is compared against a separate curve cross validated for that input pattern (residual SD 9.9 years), so no-differential readings get a fair comparison rather than being measured against full-panel expectations.

My Results: 53 Years Old With Blood That Reads 43
I ran my own numbers from my latest quarterly blood draw (I use Hundred Health, Superpower and Function Health and have reviewed all of them). At 53, my HemeAge came back 43. That lands me in the study’s resilient category, with a reading about 5 years younger than typical for my age.
The values driving it: my RDW is 12.0 percent, near the bottom of the range, and my lymphocyte count is strong. I would love to credit five days a week of strength training and a stubborn protein habit. Consistent training does support healthy blood counts! But the truth is, some of this is genetics and no single reading proves anything. What the number gives me is a baseline worth protecting and I will rerun it with every quarterly draw. Can You Change Your Blood Age?
Nobody has run a trial proving that lowering RDW extends your life. What we do know is which everyday factors move these markers and every one of them is worth doing anyway.

- RDW: iron, B12 and folate status are the big levers. RDW climbs when red blood cell production gets uneven, and nutrient shortfalls are the most common fixable cause. If yours runs high, a ferritin and B12 check is a better first move than any supplement. If your annual physical does not include those, Hundred Health, Function Health and Superpower all cover them in their panels.
- MCV: runs high with B12 or folate shortfall, heavy alcohol use and low thyroid function and runs low with iron deficiency. Alcohol deserves the callout here, it is one of the most consistent MCV raisers in otherwise healthy adults. - Red blood cell count and hemoglobin: respond to iron status, protein intake and training. This is one more place where strength training and adequate protein earn their keep. - Chronic inflammation: drives RDW up over time, which puts sleep, stress management and recovery on the blood age list too.
There is also an emerging supplement angle and it's one I've used for years now. C15:0 (pentadecanoic acid), an odd chain saturated fat found in dairy, strengthened red blood cells and reduced anemia and inflammation markers in preclinical work published in Scientific Reports and higher circulating levels track with better cardiometabolic health in humans. Worth knowing as you weigh it: much of that research comes from the scientist who founded fatty15, the company selling the pure C15:0 supplement and human trial data is still thin. I file it under promising, not proven but the evidence was interesting enough to me that I started taking it.
HemeAge vs PhenoAge vs Everything Else
If you have searched for a biological age calculator before, you have probably hit tools built on PhenoAge, the 2018 measure from Morgan Levine’s team published in Aging. PhenoAge is well validated, but it needs nine values including albumin, creatinine, CRP and alkaline phosphatase and most people do not have a recent CRP sitting around. However all of the blood test services I list on this page have all of these. That is the practical case for a CBC based tool: the inputs are on the one blood test nearly everyone already has, no matter which testing service or doctor you use.

Epigenetic tests measure something real and remain the research gold standard. For tracking over time though, the practical question is what you will actually retest. A CBC gets rerun every year whether you think about it or not, which makes it the most sustainable trend line most of us will ever have.

How is biological age calculated from a blood test?
Researchers train models on large populations to learn how blood values drift with age, then compare your values to those patterns. The HemeAge approach uses 19 standard CBC values, with RDW, MCV and red blood cell count carrying the most weight. Other blood based measures like PhenoAge add chemistry values such as glucose, CRP and albumin.
Is this calculator the same as PhenoAge?
No. PhenoAge estimates biological age from nine blood chemistry and count values and was developed by Morgan Levine’s team in 2018. This calculator follows the newer HemeAge protocol, which uses only complete blood count values, so you can run it from a standard CBC with no extra labs.
How accurate are biological age calculators?
Less precise than most people assume. Blood based age models typically miss by 15 years or more for individuals, so the value is in the direction and the trend rather than the exact number. The group level evidence is where these tools are strong: in the HemeAge study, people whose blood read more than 10 years older than their age had about triple the death risk over follow-up.
How I Keep This Guide Current
I built and tested this calculator myself in August 2026 against the published study protocol, verified every citation the week I wrote this and ran my own bloodwork through it before recommending it to you. When new validation studies on CBC based aging come out, or if the study authors release their original model, I will update both the tool and this page.
